Already using piDock? The same form is built into the app - tray icon → Contact. This is that exact form, for anyone who hasn't installed it yet (or would rather not open it right now). Prefer email? support@pidock.com.
Reporting a bug from inside piDock? Use the in-app Contact form instead - it can attach recent crash/debug logs (only with your consent) to help us fix it faster.
